Supplementary information

The questions shown in the supplementary section are specific to your chosen university or college and programme.

Please answer all the questions as fully as possible. You may be asked to select from a choice of possible answers or provide a written answer to a question.

If you have to provide a written answer you may be asked to complete the response in a text box provided. Alternatively the university may allow you to submit your response to them as an attachment. You can attach a file not exceeding 5MB by clicking on 'Attach' and then browsing to the file. To ensure that your file does not exceed the limit you could use an application such as 'winzip' or 'compress' which results in a file that is much smaller in size.

Another option is to send a document by post or by separate email. Click on 'send under separate cover' for a pop-up screen displaying the name and address of the designated recipient at the university. Your full name and Personal ID must be quoted on a covering note or in the subject line of an email.
